	STATE OF FLORIDA DEPARTMENT OF HEALTH ONSITE SEWAGE OPERATING PERMITS

Coral Ridge Commercial Condominium Association is required to pay the State of Florida Department of Health a yearly fee (2008) of $150.00 for each septic system. Late payment of this fee is associated with a significant monthly penalty. The Association has six (6) septic systems. The Association is responsible for the Operating Permit fees. The unit owners in each Phase/Building using the individual septic systems are responsible for bills associated with cleaning/pump-out of their systems/tanks. A copy of the permits paid in 2008 (to August 27, 2009) may be reviewed below. The Units/Phases associated with each septic system/permit are:

	Although the billing/mailing addresses were corrected last year, the names on the permits are confusing. The Health Department will be notified to change the name for Phase II from “COOKIES CRANE SERVICE” to “CORAL RIDGE CONDOMINIUM ASSN. UNITS 2 - 7” and change the name of Phase A - B from “THOMAS V. MALONEY” to “CORAL RIDGE COMMERCIAL CONDO ASSN. UNITS 14 - 21”
